Initial Evaluation

A Comprehensive Psychiatric Evaluation involves discussing what you are experiencing, your concerns, your mind-body symptoms, your medical and psychiatric history, family history, and psychosocial factors affecting your mental health balance and well-being.

Our philosophy is to work collaboratively with patients to come up with the best treatment plan for them and if medication is included, it is our goal to treat with the lowest effective dose of medication.

Our psychiatric treatment team consisting of Psychiatrists (MDs) and Psychiatric Mental Health Nurse Practitioners (PMHNP) will work collaboratively with your therapist at Fusion Care to develop a holistic treatment plan.

Establishing Diagnoses

Information gathered in the initial evaluation (including questionnaires you complete before your visit) will be thoroughly reviewed to establish an accurate DSM-5 TR diagnosis to guide treatment. Common DSM-5 TR diagnoses include but are not limited to, Depressive Disorder, Anxiety Disorder, Bipolar Disorder, PTSD (Post Traumatic Stress Disorder), Obsessive-Compulsive Disorder, Insomnia Disorder, ADHD (Attention Deficit and Hyperactivity Disorder), and Personality Disorder.

Developing a Treatment Plan

Based on your DSM-5 TR diagnosis, and presenting symptoms, together we will develop an individualized treatment plan to address your unique personal needs and goals.

Your individualized treatment plan can include Psychotropic Medication(s), Complementary, Alternative, and/or Psychotherapeutic Treatment Modalities, laboratory testing, recommendations for further Diagnostic Imaging/Testing, and referrals to Primary Care Physicians and Psychiatric Specialists.

We can discuss Pharmacogenomic testing, at your request.
